Durable Power of Attorney for Health Care
Hired by client, family, or Conservator as an on-going health and social advocate. The appointment of DPOA is made by a competent individual to act as their agent under their Power of Attorney and/or Health Care Directive. Services are tailored to meet their specific needs and wishes. This can also include all the Client Services listed below.
Case Management
Hired by client, family, or Conservator as an on-going health and social advocate. Excellent supplemental service for those court appointed Guardians, either local or out-of-state, on an urgent, temporary, or long-term basis. This can also include all the Client Services listed below.
Consulting and Assessment
Review client and family needs and concerns. Review possible options including referrals and resource information as needed. Emergent, transitional short-term phone or in-person consultation, or care coordination. Offering presence at medical appointments or procedures. Written Personalized Plan of Action available. This can include all of the Client Services listed below.

As appropriate for the benefit and need of the client including all aspects of advocacy, care management, consents or authorizations, service enlistment, communication and coordination in or for:
